The Department of Public Health in Western Massachusetts is seeking a part-time Nurse Practitioner (NP) to provide culturally competent medical care to a diverse patient population. The role involves clinical assessment, treatment planning, and collaboration with healthcare teams, primarily in a long-term care setting. The position includes at-home on-call responsibilities and emphasizes public health and community engagement.

Responsibilities

• Evaluate and examine patients, including taking medical histories, conducting physical examinations, and ordering tests.
• Prescribe medications and treatments as appropriate.
• Collaborate with physicians and healthcare professionals to coordinate patient care and serve as a liaison between nursing and medical staff.
• Participate in the Rehab Plan of Care (RPOC) meetings for new patients, providing medical assessments to facilitate care planning.
• Ensure timely documentation of medical records, progress notes, and interventions.
• Participate in home visits to assess family dynamics and home environment for continuity of care.
• Develop and implement individualized care plans and serve as a team leader in patient meetings.
• Contribute to quality assurance activities, data collection, and policy development.

Compensation

• Salary-based compensation, with placement determined by experience and education.
• W-2 tax status.

Shift & Schedule

• 20 hours per week, primarily on Mondays (8:00 am - 6:00 pm) and Thursdays (6:30 am - 4:30 pm).
• At-home on-call responsibilities for a minimum of one weekday night and one weekend per month.

Requirements

• Current and valid registration as a professional nurse in Massachusetts.
• Authorization to practice as a Nurse Practitioner in Massachusetts.
• At least three years of full-time experience as a registered nurse in a recognized healthcare setting, or an equivalent combination of experience and education.
• Experience with physical diagnosis techniques, including medical history taking and physical evaluations.
• Knowledge of safety practices in medical facilities.
• Ability to establish rapport with patients from diverse backgrounds.
• Proficiency in Microsoft Office applications (Excel, Outlook, Word).
• Strong communication skills and ability to work independently with physicians and administrative staff.
• Experience managing care for patients with ventilators, tracheostomies, and G-tubes (preferred).
