The Advanced Practice Nurse (APN) provides primary care in collaboration with physicians and the healthcare team, focusing on home-based, facility-based, and palliative care patients. The role involves independent and team-based patient management, including assessments, care planning, and education, with a focus on hospice and palliative services.

Responsibilities

• Obtain patient histories and perform physical examinations; prescribe appropriately based on clinical findings.
• Collaborate with interdisciplinary team members to deliver holistic patient care.
• Coordinate with primary physicians to develop care plans for palliative and hospice patients.
• Manage care of patients in home, hospital, hospice inpatient units, or nursing homes, establishing visit schedules.
• Maintain and develop referral relationships with community physicians and healthcare systems.
• Provide leadership in creating and developing specialized hospice and palliative care services.
• Participate in ongoing learning, self-development, and facilitate education for team members.
• Support compliance with organizational policies, legal guidelines, and participate in quality improvement initiatives.

Requirements

• Graduate of an accredited nurse practitioner program (ARNP).
• Unrestricted license to practice in Iowa.
• Current DEA certificate without restriction.
• Current CPR certification.
• Valid driver's license, auto liability insurance, and reliable transportation if driving is required.
• Ability to maintain confidentiality and protect sensitive data.
• Strong interpersonal, communication, and collaboration skills.
• Ability to handle emotional stress and multitask effectively.
• Pre-employment physical, drug screen, TB test, and background check completed.
• Preferred: at least 2 years of related experience, hospice or palliative care experience.
