Responsibilities

• Evaluate and assess patients' health history and physical condition.
• Diagnose medical conditions and develop individualized care plans.
• Manage health problems by prescribing medications or treatments.
• Coordinate and collaborate with patients, families, and healthcare practitioners.
• Perform face-to-face assessments and evaluate hospice eligibility in consultation with the interdisciplinary team.
• Visit patients in their homes and community settings to deliver hospice services.
