This position involves providing medical care at a skilled nursing facility that offers long-term care, subacute rehabilitation, and therapy services. The role includes on-site clinical duties with telehealth support, and offers flexible scheduling with part-time and full-time options. Compensation is based on a salary plus RVU productivity bonuses, with additional benefits provided.

Responsibilities

• Provide medical care to long-term, subacute, and rehabilitation patients at a skilled nursing facility.
• Maintain a minimum of two on-site workdays per week, with telehealth support available for virtual consultations three days per week.
• Participate in the on-call rotation approximately once every 4-6 weeks, with the option to join the company-wide on-call pool.
• Collaborate with multidisciplinary teams including physical, occupational, and speech therapists.
• Document patient encounters and treatment plans in accordance with facility policies.
• Support patient recovery through personalized orthopedic, cardiac, and post-surgical rehabilitation programs.
• Work at both facility locations as needed.
• Engage in virtual interactions and consultations via telehealth platforms.

Compensation

• $110,000 to $225,000 base salary plus RVU productivity bonuses
• Compensation model based on wRVU (productivity-based)
• Additional income support includes relocation assistance and productivity bonuses

Requirements

• Medical degree (MD or DO) or appropriate licensure for Nurse Practitioners or Physician Assistants.
• Current license to practice medicine in the state of New York.
• Experience in long-term care, rehabilitation, or related clinical settings preferred.
• Ability to work independently and as part of a multidisciplinary team.
• Excellent communication and documentation skills.
• Comfort with telehealth technology and virtual patient interactions.
• Willingness to participate in on-call rotation if desired.
• Strong commitment to patient-centered care.
