The Nurse Practitioner will conduct comprehensive home risk assessments and adult/geriatric evaluations across various community settings within Ohio and neighboring states. This full-time, traveling role involves collaborating with healthcare teams to review patient health, medical history, and social factors, aiming to improve care quality and address care gaps. Responsibilities

  • Coordinate with Primary Care Physicians on patient education and follow-up care.

  • Review and observe a member's health, medication adherence, social environment, and risks during visits.
  • Assist in closing care gaps through comprehensive assessments and follow-up.
  • Maintain timely and accurate documentation of all clinical encounters.

Compensation

  • Salary range of USD 113,700 to USD 133,500 annually based on experience.
  • Compensation model includes salary plus incentives.
  • Sign-on bonus of $7,500.
  • W-2 tax status.

Shift & Schedule

Requirements

  • Master's Degree in Nursing or equivalent experience with NP licensure.
  • Current RN and Nurse Practitioner licensure in Ohio, including prescriptive authority or ability to obtain it.
  • Board certification by AANP, ANCC, or AACCN in an approved specialty.
  • Current BLS, ACLS, or CPR certification.
  • At least 1 year of active Nurse Practitioner experience.
  • Proficiency with electronic health records and medical software.
  • Excellent verbal and written communication skills.
  • Ability to adapt to frequent changes and travel extensively within the state and neighboring states.
  • Valid driver's license, reliable vehicle, and proof of automobile insurance.
