We are seeking a licensed Nurse Practitioner to conduct cognitive, functional, and behavioral assessments via telehealth or in-clinic settings. The role involves developing individualized care plans, interpreting assessment results, and collaborating with primary care providers to support early detection and management of cognitive impairments. This position offers flexible, mostly remote work with responsibilities aligned to regulatory standards and documentation requirements.

Responsibilities

• Utilize BrainCheck cognitive screening tools and other assessments to evaluate patient status and inform care planning.
• Administer the BrainCheck Assess, ensuring patient understanding and completion within validity parameters.
• Conduct screenings for anxiety and depression.
• Perform functional assessments of daily living activities and safety considerations.
• Develop personalized care plans addressing cognitive, neuropsychiatric, and functional issues.
• Educate patients and families about cognitive disorders, including diagnosis, prognosis, and treatment options.
• Communicate with treatment team members to ensure coordinated care and optimal patient outcomes.
• Document supervision time and patient participation according to CMS and payer guidelines.

Requirements

• Strong clinical assessment skills, particularly in cognitive evaluations.
• Proficiency with telehealth platforms and digital health technologies.
• High attention to detail and ability to meet structured documentation timelines.
• Commitment to HIPAA compliance and patient privacy.
• Excellent communication skills for patient education and team collaboration.
• Spanish language skills are a plus.
