Journey Hospice is seeking a Per Diem Nurse Practitioner to provide medical and palliative care services to hospice patients in Essex County. The role involves performing face-to-face patient assessments to certify eligibility for Medicare Hospice Benefit, documenting clinical encounters, and collaborating with the interdisciplinary team under the supervision of the Hospice Medical Director.   • Ability to provide timely, professional services in compliance with policies and legal standards.
  • Effective communication skills and a compassionate attitude toward hospice patients.
