Journey Hospice is seeking a Per Diem Nurse Practitioner to provide medical and palliative care services to hospice patients in Essex County. The role involves performing face-to-face patient assessments to certify eligibility for Medicare Hospice Benefit, documenting clinical encounters, and collaborating with the interdisciplinary team under the supervision of the Hospice Medical Director. This position offers flexible hours without guaranteed shifts.

Responsibilities

• Perform patient medical examinations and palliative care visits in accordance with organizational policies and federal/state regulations.
• Conduct face-to-face encounters required for Medicare Hospice Benefit certification after six months of hospice care.
• Document visits and face-to-face encounters accurately in the patient's clinical record and required forms.
• Provide medical care and palliative interventions for hospice patients on an as-needed basis.
• Communicate effectively with the Medical Director, Administrator, and interdisciplinary team members.
• Ensure compliance with hospice policies, billing procedures, and regulatory requirements.
• Maintain professional and compassionate interactions with patients, caregivers, and team members.
• Develop and sustain positive relationships with healthcare professionals and the community.

Compensation

Requirements

• Graduation from a master's-level program for nurse practitioners or clinical nurse specialists.
• Completion of a graduate-level 3-credit Pharmacology course or a 45-hour integrated pharmacology course within the master's program.
• Current licensure and national certification as an Advanced Practice Nurse in New Jersey (N.J.A.C. 13:37-7.1).
• Valid driver's license and reliable, insured transportation in accordance with New Jersey requirements.
• Minimum of two (2) years of experience as a Nurse Practitioner.
• Understanding of hospice care programs and regulations.
• Ability to provide timely, professional services in compliance with policies and legal standards.
• Effective communication skills and a compassionate attitude toward hospice patients.
• Demonstrates professionalism, teamwork, and a positive demeanor.
