The Physician Assistant will provide comprehensive medical care to chronically ill geriatric members within a senior care program, collaborating with interdisciplinary teams to promote independence and quality of life. The role involves conducting assessments, managing treatment plans, and educating staff, members, and families, with a focus on proactive and patient-centered care.
Responsibilities
- Perform health histories and comprehensive medical examinations for new members to establish initial care plans.
- Initiate, evaluate, and review medical orders, identifying opportunities for care improvement.
- Perform procedures and document all care activities and recommendations for team members.
- Order, interpret, and follow up on laboratory tests and results.
- Coordinate consultations with medical and non-medical providers, including specialists, therapists, and social workers.
- Educate staff, members, and caregivers on essential care functions to maximize independence at home and in care centers.
- Conduct ongoing assessments to update and modify care plans based on evaluations and member feedback.
- Collaborate closely with interdisciplinary team members to ensure effective, timely, and innovative care delivery.

Requirements
- Current Physician Assistant licensure and certification.
