Community Healthcare of Texas is seeking a Palliative Advanced Practice Nurse to provide hospice and palliative care services under the supervision of the Chief Medical Officer. The role involves collaborating with physicians and care teams to deliver symptom management, document patient progress, and support patients with serious and terminal illnesses within North Central Texas.

Responsibilities

• Document medical history, patient information, hospice eligibility, disease progression, medications, and diagnostic studies, and communicate findings to the Medical Director.
• Consult with the Medical Director or designee when care plans fall outside standard protocols or require specialty input.
• Review, follow, and update the written plan of care in coordination with the interdisciplinary team.
• Ensure patients receive appropriate symptom control measures through collaboration with team members.
• Document care provided, including disease progression, in the patient's clinical record.
• Attend physician meetings and other relevant meetings as directed.
• Establish and maintain relationships with staff and community referral sources to support program growth.
• Participate in quality assurance and performance improvement processes.

Compensation

• Sign-on bonus of $7,500 (taxable W-2 income).

Requirements

• Licensed as an Advanced Practice Nurse in Texas, with current certification from an approved certifying body (e.g., AANP, ANCC, HPNA ACHPN).
• Minimum of 3 years of nursing experience, preferably at the APN level.
• Proficient in medicine, pharmacology, pain and symptom management, and hospice principles.
• Valid Texas Driver's License and proof of automobile liability insurance.
• Proficient in computer skills, including Microsoft Office suite.
• Knowledge of regulations pertaining to palliative and hospice care for all payers.
• Ability to perform essential duties satisfactorily with reasonable accommodations if needed.
