The Nurse Practitioner will provide comprehensive patient care within a hospice setting, including physical assessments, care planning, medication management, and progress monitoring. The role involves collaboration with a multidisciplinary team to deliver end-of-life care aligned with patient needs and goals.

Responsibilities

• Record physical findings, formulate care plans and prognosis based on patient condition.
• Discuss cases with physicians and other health professionals to develop comprehensive care plans.
• Prescribe or recommend medications and other treatments.
• Monitor and document patient progress, reporting updates to supervising physicians.
• Collaborate with hospice staff and volunteers as part of an interdisciplinary team.
• Assist in lifting and transferring patients as required, with the ability to lift objects over 20 pounds.
• Perform duties requiring close and distance vision, focus adjustment, and physical mobility including standing, walking, stooping, kneeling, and crouching.
• Work variable schedules, including day or night shifts, weekdays or weekends.

Requirements

• Minimum of 2 years of experience in hospice, home health, or community health within the past five years.
• Completion of an accredited Nurse Practitioner program.
• Current state licensure as a Nurse Practitioner.
• Ability to assist in patient transfers and lift objects over 20 pounds.
• Specific vision abilities including close and distance vision and focus adjustment.
• Physical ability to stand or walk for over one hour per day, and perform necessary movements such as stooping, kneeling, and crouching.
• Excellent communication and collaboration skills within a multidisciplinary team.
• Experience working in end-of-life or hospice care preferred.
• Ability to work flexible schedules, including variable days and shifts.
