The Nurse Practitioner (NP) provides direct patient care in various settings including medical centers, nursing homes, skilled nursing facilities, and home care. The role involves conducting assessments, developing treatment plans, ordering diagnostics, prescribing medications, and collaborating with a multidisciplinary team to deliver comprehensive geriatric care. The NP functions independently within a written practice agreement and adheres to regulatory and organizational standards to ensure quality patient outcomes.

Responsibilities

• Conduct comprehensive geriatric assessments, including medical history, physical exams, and diagnosis.
• Develop, document, and implement patient management and nursing care plans tailored to the patient population.
• Order and interpret laboratory and diagnostic studies; prescribe medications within scope of practice.
• Refer patients to specialists and coordinate care transitions, including discharges from SNF and home settings.
• Participate in clinical rounds, conferences, and documentation of progress notes and summaries.
• Collaborate with physicians, nurses, social workers, and other healthcare professionals to optimize patient care.
• Recognize urgent situations requiring immediate physician intervention and initiate life-saving procedures if necessary.
• Educate patients and families about treatment plans, health management, and post-discharge needs.

Requirements

• Current Nurse Practitioner certification in the state of practice.
• Board certification by AANP or ANCC preferred; required in some states.
• Current DEA registration if prescribing controlled substances.
• Minimum of 1 year of clinical experience preferred; strong new graduates considered.
• Basic Life Support (BLS) certification from AHA or Red Cross within first 90 days.
• Ability to perform physical assessments, develop care plans, and document thoroughly.
• Strong critical thinking, problem-solving, and communication skills.
• Empathy, attention to detail, and ability to work independently and in team settings.
• Basic computer skills and familiarity with Microsoft Office Suite.
• Willingness to travel 50-75% locally as required.
