We are seeking experienced Nurse Practitioners to join our home care and outpatient programs in the West Coast Florida area. The roles involve providing in-home care for high-risk, frail patients, as well as participating in Medicare Advantage and Medicaid programs. The positions require clinical expertise in outpatient and Medicare populations, with a focus on patient assessment and management.
Responsibilities
- Conduct in-home visits for high-risk, frail patients, including vital sign assessment and patient management.
- Manage a patient panel of approximately 6-8 patients per day in the high-intensity program.
- Perform clinical assessments and provide treatment for adult patients, including those with Medicare Advantage and Medicaid.
- Participate in new program initiatives, including the Medicare Advantage and ObamaCare programs.
- Document patient encounters accurately and maintain compliance with program protocols.
- Coordinate with healthcare teams and ensure continuity of care for assigned patients.
- Complete all required documentation and reporting in a timely manner.
- Maintain current licensure and stay updated on relevant clinical guidelines.
Compensation
- $110,000 – $120,000 yearly salary
- Performance bonuses available based on experience and performance
- W-2 employment with tax withholdings
Shift & Schedule
- Full-time position with a flexible schedule based on patient visits
- Roles to be filled by March 31, 2023
Requirements
- Valid Nurse Practitioner license in Florida.
- Minimum of 3 years' outpatient clinical experience, particularly with Medicare Advantage populations.
- Experience managing high-risk or frail patients in a home or outpatient setting preferred.
- Ability to perform comprehensive patient assessments and vital sign monitoring.
- Strong documentation and communication skills.
- Self-motivated with the ability to work independently in a home care environment.
- Reliable transportation and ability to travel to patient homes within designated areas.
- Knowledge of Medicare, Medicaid, and ObamaCare programs is preferred.
- Ability to meet deadlines and work efficiently in a fast-paced environment.
- No medical assistants are involved; the NP will perform all clinical duties.
