Elara Caring is seeking a Hospice Nurse Practitioner to provide comprehensive palliative and hospice care in patients' homes. The role involves conducting face-to-face recertification visits, managing patient symptoms, supporting families, and collaborating with medical teams to deliver high-quality end-of-life care. Responsibilities

  • Complete routine and emergency face-to-face recertification visits, including admissions, transfers, and extended stays.
  • Record observations, treatments, and pertinent patient information.
  • Communicate findings and care measures with Medical Director, Attending Physician, and interdisciplinary team.
  • Provide physical and emotional support to patients and families during crises.
  • Triage consults based on patient acuity and perform bedside palliative care with education.
  • Ensure activities align with organizational standards and prevent unnecessary hospitalizations.
  • Maintain accurate patient records, charts, and documentation for medical and billing purposes.
  • Collaborate with medical and administrative teams and assist in developing clinical guidelines.

Requirements

  • Master's degree in nursing and certification from a national Nurse Practitioner licensing board.

  • Ability to travel approximately 50% of the time.
  • Physical ability to sit, stand, bend, lift, and move intermittently, including lifting 50-100 lbs.
  • Valid driver's license, insurance, and reliable transportation.
  • Knowledge of CMS billing and coding guidelines.
  • Strong communication and collaboration skills.
  • Compliance with applicable laws and regulations regarding collaborative agreements.
