Theoria Medical is seeking full-time Physician Assistants and Nurse Practitioners to provide high-quality, patient-centered primary care at skilled nursing facilities. The role involves managing clinical care, performing daily rounds, prescribing treatments, and documenting encounters in an electronic health record system. Responsibilities

• Manage clinical care and perform daily rounds for residents at assigned skilled nursing facilities.
• Prescribe medications and treatments based on patient needs.
• Provide comprehensive primary care services to SNF residents.
• Document all patient encounters accurately and promptly in the electronic health record system (ChartEasy, PointClickCare, MatrixCare, or American HealthTech).
• Be available for phone or secure messaging consultations with facility nursing staff.
• Attend scheduled in-person shifts on time and on-site as required.
• Ensure timely documentation and communication with the care team.
• Maintain reliable transportation to and from facilities.

Compensation

• Salary based with monthly productivity bonuses.
• Compensation includes a $1,500 CME and conference allowance.

Shift & Schedule

• Monday through Friday, 6-hour shifts from approximately 7AM to 1PM local time.
• No overnight shifts or on-call responsibilities.
• Shifts are in-person with flexibility within reason.
• Documentation can be completed off-site if needed.

Requirements

• Current certification as a Physician Assistant or Nurse Practitioner.
• Ability to perform clinical rounding and manage patient care in a skilled nursing facility setting.
• Proficiency with electronic health records such as ChartEasy, PointClickCare, MatrixCare, or American HealthTech.
• Reliable transportation and punctuality for scheduled shifts.
• Excellent communication skills for collaboration with nursing staff and patients.
• Ability to work independently within a team environment.
• Flexibility to work Monday through Friday, 6-hour shifts, with availability during morning hours (7AM-1PM).
• Licensure in the state of practice (reimbursed by employer).
• Ability to perform the essential functions of the role with or without reasonable accommodation.
