A community health center is seeking an experienced Family Nurse Practitioner (FNP) or Family Physician Assistant (PA) to provide outpatient primary care services. The role involves managing a diverse patient population across all ages in a full-time, hospital-based outpatient setting with a standard Monday through Friday schedule and occasional weekend and after-hours call duties.

Responsibilities

• Provide primary care services to patients of all ages in an outpatient setting.
• Manage 20-22 patients per day with 15-30 minute appointments.
• Participate in scheduled weekend and after-hours call rotations (1 weekend and 1 week every 17 weeks).
• Perform comprehensive assessments, diagnose, and develop treatment plans.
• Coordinate patient care and follow-up in a team-based environment.
• Complete administrative documentation within designated hours.
• Contribute to community health initiatives and patient education.
• Maintain compliance with clinical protocols and standards.

Shift & Schedule

• Full-time, Monday through Friday, 9:00 AM to 5:00 PM.
• Occasional Saturday work once every 13 weeks (9:00 AM to 12:00 PM).
• Call responsibilities include 1 weekend and 1 week every 17 weeks, with schedule varying.

Requirements

• MD or DO degree required.
• Minimum of 2 years of direct Family Practice experience.
• Current licensure to practice in the state.
• Ability to see all ages and manage a high-volume outpatient caseload.
• Excellent communication and clinical skills.
• Experience working in a hospital or clinic setting preferred.
• Willingness to participate in call rotation and community health activities.
