Hearth Hospice is seeking Per Diem Hospice Nurse Practitioners to provide comprehensive end-of-life care in the Chattanooga area. The role involves evaluating patients, developing care plans, collaborating with healthcare teams, and supporting patients and families through hospice services. The position offers flexible scheduling with an average of 10 face-to-face visits per week, under physician supervision.

Responsibilities

  • Provide comprehensive medical care, including palliation and management of terminal illnesses, in compliance with state Nurse Practice Act and under physician supervision.
  • Consult with attending physicians, hospice medical director, and healthcare practitioners to assess patient eligibility and ongoing care needs.
  • Review clinical information and coordinate patient care with the interdisciplinary team to ensure holistic, patient-centered services.
  • Educate and counsel patients and families on medical, psychological, and spiritual concerns, collaborating with the team to meet care goals.
  • Make recommendations regarding patient eligibility for initial certification and recertification for hospice care to the Hospice Medical Director.
  • Maintain current knowledge of Medicare/Medicaid, state, and federal hospice regulations to ensure compliance and high-quality care.
  • Deliver compassionate care tailored to terminally ill patients, managing symptoms and promoting comfort.
  • Collaborate with healthcare providers to ensure seamless care coordination and communication.

Compensation

  • Paid on a fee-for-service basis at $125 per face-to-face visit.
  • W-2 employment status.

Requirements

  • Master's degree; hospice or palliative care experience as an RN or NP preferred.
  • Current unencumbered license as a Registered Nurse and Nurse Practitioner in the state of practice.
  • Certification in a specialty area such as Hospice and Palliative Nursing (CHPN), Pediatrics, or Geriatrics preferred.
  • Excellent verbal and written communication skills.
  • Proficiency in electronic medical records (EMR) and computer use.
  • CPR certification if required by state.
  • Ability to work independently and collaboratively in a team environment.
  • Knowledge of hospice regulations and compliance requirements.
  • Experience in patient education and counseling.
  • Ability to evaluate and manage complex medical conditions in a hospice setting.
