The Per Diem Palliative Nurse Practitioner provides medical and palliative care services to hospice patients in accordance with organizational policies, state and federal regulations, and under the supervision of the Hospice Medical Director. This role involves performing face-to-face patient assessments to certify eligibility for Medicare Hospice Benefit and collaborating with the interdisciplinary team to ensure quality patient care.

Responsibilities

• Perform patient medical examinations and palliative care visits for hospice patients.
• Conduct face-to-face encounters required for Medicare Hospice Benefit certification after six months of hospice care.
• Document visits and face-to-face encounters in the patient's clinical record and on required forms.
• Provide medical care services and palliative care interventions on an as-needed basis.
• Communicate effectively with the Medical Director, Administrator, and interdisciplinary team members.
• Adhere to all applicable policies, procedures, and regulations related to hospice care and billing.
• Maintain professional and compassionate interactions with patients, caregivers, and team members.
• Ensure compliance with state and federal healthcare laws and hospice standards.

Compensation

• Compensation model classified as 'Other' with W-2 tax status.
• Per diem position with no guaranteed hours.

Requirements

• Master's degree in Nursing from an accredited program for nurse practitioners or clinical nurse specialists.
• Completion of a graduate-level 3-credit Pharmacology course or a 45-hour integrated pharmacology course within the Master's program.
• Current licensure and national certification as an Advanced Practice Nurse through the Pennsylvania Board of Nursing.
• Valid driver's license and reliable, insured transportation in accordance with Pennsylvania requirements.
• Minimum of two (2) years of experience as a Nurse Practitioner.
• Understanding of hospice care programs and regulations.
• Ability to communicate effectively and maintain a professional demeanor.
• Demonstrates compassion and a patient-centered approach in hospice settings.
• Ability to work independently and collaboratively within a team.
• Knowledge of Medicare Conditions of Participation and compliance standards.
