Infinite Medical P.C. is seeking a part-time Primary Care Nurse Practitioner or Physician Assistant to provide on-site primary care services in long-term care facilities in Gahanna, Ohio. The role involves managing residents' health through exams, diagnoses, treatment plans, and collaboration with facility staff, emphasizing elder care and patient-centered approaches.

Responsibilities

• Provide primary care services to residents in skilled nursing and long-term care facilities
• Perform physical exams and medical history assessments
• Diagnose and treat chronic and acute health conditions
• Order and interpret diagnostic tests and lab results
• Develop and adjust individualized care plans
• Prescribe medications and monitor treatment effectiveness
• Collaborate with facility staff and interdisciplinary teams
• Educate patients and families about treatment options and care goals

Compensation

• Salary range: $90,000 - $120,000 per year
• Compensation model: W-2 salary
• Additional income: Sign-on bonus

Requirements

• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license
• State-specific license in good standing in Ohio
• Minimum 6 months of experience in long-term care or skilled nursing facilities
• Strong communication, clinical, and critical thinking skills
• Passion for elder care and person-centered healthcare
• Ability to lift at least 20 pounds and stand/walk/work on feet up to 6-8 hours per day
