The Nurse Practitioner (NP) at CRHS is responsible for providing comprehensive wound care and preventive services across all age groups, including inpatient, nursing home, and outpatient settings. The role involves developing and implementing wound care programs, educating staff, and collaborating with the healthcare team to ensure quality patient care aligned with organizational goals.

Responsibilities

• Develop and implement CRHS Wound Care Program policies, procedures, and protocols.
• Perform skin assessments and early skin care interventions to prevent pressure ulcers and promote wound healing.
• Create individualized care plans for maintaining skin integrity, healing wounds, and ensuring patient comfort.
• Coordinate wound care efforts across the CRHS continuum to reduce service duplication and improve patient satisfaction.
• Provide staff training and education on wound care practices.
• Oversee inpatient and nursing home wound care services, including rounds and consultations.
• Deliver direct clinical care to patients in inpatient, nursing home, and outpatient settings.
• Participate in performance improvement projects and assist physicians with epidemiological issues.

Compensation

Requirements

• Master's Degree in Nursing.
• Two to four years of relevant work experience or equivalent education, training, and experience.
• Current state license as a Registered Nurse.
• Current state certification as a Nurse Practitioner (NP).
• Current Basic Life Support (BLS) certification or obtain within three months of hire.
• Preferred certification in wound care.
• Ability to provide clinical care across diverse settings and age groups.
• Strong communication and team collaboration skills.
• Knowledge of wound care protocols and prevention strategies.
• Commitment to ongoing professional development and compliance with licensure requirements.
