Mend Health is seeking a Mobile Wound Care Nurse Practitioner to provide specialized wound management services in patients' homes across Ohio. The role involves assessing, treating, and educating patients with acute and chronic wounds, collaborating with healthcare teams, and ensuring high-quality, patient-centered care in a home setting.

Responsibilities

• Conduct comprehensive wound assessments, including measurements and tissue viability evaluations.
• Develop and implement individualized wound care plans based on assessments and evidence-based practices.
• Perform wound debridement, dressing changes, and other wound care procedures using sterile techniques.
• Educate patients, caregivers, and families on wound care management, prevention, and signs of complications.
• Collaborate with physicians and healthcare providers to coordinate patient care and optimize outcomes.
• Maintain accurate medical records, including assessment documentation, treatment plans, and progress notes.
• Participate in ongoing education to stay current with advancements in wound care.
• Adhere to organizational policies, procedures, and regulatory standards to ensure patient safety.

Requirements

• 3-5 years of clinical experience as a Nurse Practitioner.
