Healthy Community Health Centers (HCHC) is seeking a licensed Family Nurse Practitioner to provide primary care services within a community health setting. The role involves delivering comprehensive adult and pediatric care, managing chronic conditions, and supporting wellness and preventive health initiatives. Responsibilities

• Provide primary care services to adult and pediatric patients, including acute and chronic disease management.
• Collaborate with a multidisciplinary team to coordinate patient care and community resources.
• Maintain accurate and timely documentation in electronic medical records (EMR).
• Support wellness, preventive, and women's health services, including pre- and post-natal care.
• Participate in clinical workflows and strategies to ensure high-quality patient care.
• Adhere to all applicable licensing, certification, and regulatory requirements.
• Engage in community outreach and health education initiatives as needed.
• Support organizational goals of providing accessible, patient-centered care.

Requirements

• Virginia State License with independent practice authority.
• Current unrestricted Virginia medical license or eligibility for one.
