Hosparus Health is seeking a Nurse Practitioner to provide medical care for patients facing advanced illness or end-of-life situations. The role involves managing chronic and acute conditions, performing procedures, and collaborating with a multidisciplinary team within a hospice and palliative care setting. The position is based at Nazareth Home Highlands and requires working Monday through Friday with on-call responsibilities.

Responsibilities

• Plan, direct, and evaluate medical care for assigned patients in designated facilities.
• Examine and treat chronic and urgent episodic illnesses, injuries, and accidents.
• Perform selected laboratory tests and make referrals to physicians as needed.
• Write prescriptions and perform procedures within the scope of expertise and protocols.
• Collaborate with the facility's medical director, attending physicians, and care team members to provide comprehensive patient care.
• Work in accordance with on-call duty policies.
• Utilize electronic medical records and other technology-based tools effectively.
• Provide care in a holistic, team-based approach aligned with hospice and palliative care standards.

Requirements
