Address Healthcare of Michigan is seeking a part-time licensed Primary Care APRN or PA to provide home-based primary care in the Grand Rapids, MI area. The role involves performing age-appropriate assessments, managing acute and chronic conditions, and collaborating with a multidisciplinary team to deliver comprehensive care to homebound and assisted living patients. Responsibilities

• Perform age-appropriate history and physical examinations for complex, acute, critical, and chronically ill patients independently.
• Conduct annual wellness exams.
• Diagnose and develop treatment plans for homebound patients with acute, episodic, or chronic illnesses by analyzing medical histories, symptoms, physical findings, and diagnostic imaging.
• Maintain standards of care and adhere to professional practice guidelines.
• Collaborate with specialists to develop treatment plans for high-risk patients.
• Partner with local home health agencies, labs, imaging providers, and vendors.
• Communicate with patients and caregivers regarding self-management of diagnoses.
• Respond to patient and caregiver questions in a timely and knowledgeable manner.
