The Local Health Guide is a licensed Nurse Practitioner providing primary and urgent care to individuals with serious mental illness in community settings. The role involves conducting comprehensive assessments, managing chronic conditions, delivering urgent care, and collaborating with a multidisciplinary team to support holistic, person-centered treatment plans for a vulnerable population.

Responsibilities

  • Conduct in-person assessments of behavioral and physical health conditions, including history-taking and physical examinations.
  • Order and interpret diagnostic studies to confirm or rule out diagnoses.
  • Provide primary and urgent care to manage conditions such as COPD exacerbations, uncontrolled diabetes, CHF exacerbations, infections, pain, anxiety, depression, and suicidal ideation.
  • Collaborate with peer recovery specialists, behavioral health providers, and other team members to develop and implement individualized recovery and care plans.
  • Make referrals to internal and external programs or providers based on assessed needs.
  • Support individuals in developing self-management skills and person-centered care through motivational interviewing and holistic planning.
  • Organize clinical documentation to support appropriate record-keeping and disability documentation.
  • Work in home, street, or community-based settings, maintaining professional boundaries and engagement skills.

Compensation

  • $142,500 annual salary.
  • Compensation includes base salary, potential performance bonuses, and equity or incentive programs for clinical roles.
  • W-2 employment tax status.
  • Additional income opportunities such as performance bonuses and equity incentives.

  • 16 weeks parental leave for either parent.

  • Experience working with populations facing behavioral health and substance use challenges.
  • Strong interpersonal, communication, and listening skills with an empathetic approach.
  • Comfort with home and community-based visits and non-traditional work environments.
  • Proficiency with electronic health records and care management software.
  • Ability to adapt to dynamic assignments and interdisciplinary team collaboration.
  • Commitment to providing compassionate, person-centered care.
  • Interest or experience in working with individuals with serious mental illness and complex healthcare needs.
