Grayce is seeking licensed Nurse Practitioners or Physician Assistants to serve as Virtual Care Providers, delivering telemedicine-based assessments and care planning for Medicare beneficiaries. This remote, independent contractor role involves conducting comprehensive evaluations, supporting care navigation, and collaborating with interdisciplinary teams to provide whole-person, patient-centered care tailored to older adults and individuals with complex health needs.
Responsibilities
- Conduct comprehensive telemedicine-based evaluation and management (E&M) visits to establish care, assess medical conditions, identify barriers to care, and develop care plans for Medicare recipients.
- Review and approve care plans created by Care Navigators, ensuring clinical appropriateness and individualization.
- Document clinical assessments and findings clearly and efficiently in compliance with Medicare billing standards and telemedicine best practices.
- Partner with Care Navigators and interdisciplinary team members to support whole-person care that addresses medical, emotional, and social needs.
- Provide clinical leadership and guidance to ensure continuity of care and support for complex cases.
- Stay informed on telemedicine best practices and evolving Medicare care models, contributing insights to improve service delivery.
- Support social determinants of health by addressing social drivers impacting medical outcomes.
- Work remotely with flexible scheduling, maintaining independence as an independent contractor.

Requirements
- Active NP or PA license in good standing, with multi-state licensure in 30-50 states.

- Knowledge of Medicare guidelines, chronic condition management, and social determinants of health.
- Active enrollment as a Medicare provider (preferred).
- Experience using telehealth platforms and familiarity with digital health products.
- Strong communication skills and ability to collaborate with interdisciplinary teams.
