The Nurse Practitioner (NP) at PACE Southeast Michigan provides medical care to program participants in collaboration with physicians and the interdisciplinary team. The NP conducts assessments, manages acute and chronic conditions, facilitates care coordination, and participates in program activities, including after-hours on-call duties. The role involves working across various settings such as clinics, homes, hospitals, and long-term care facilities, with responsibilities aligned with state licensure and clinical practice standards.

Responsibilities

  • Perform periodic assessments, physical examinations, and evaluate episodic illnesses for PACE participants.
  • Collaborate with physicians and interdisciplinary team members to develop and implement care plans.
  • Make referrals to specialists and coordinate primary care services, including health promotion and screening.
  • Document all clinical activities accurately and timely in the medical record.
  • Facilitate hospital and facility admissions, participate in care planning, and communicate participant changes to the team.
  • Provide education to participants, families, and staff regarding health issues and advance directives.
  • Diagnose and treat common health problems in collaboration with physicians, including ordering diagnostic tests and adjusting treatment plans.
  • Participate in committee activities, quality assurance projects, and professional development within PACE SEMI and external organizations.

Compensation

  • Salary-based compensation (W-2 tax status).

Shift & Schedule

  • After-hours on-call medical assistance on a rotating basis via telephone triage or home visits.

Requirements

  • Current Michigan Registered Nurse license and Nurse Practitioner license with a Master of Science in Nursing (clinical focus).
  • Three to five years of clinical experience as an RN, with at least one year working with a frail or elderly population.
  • Ability to establish and maintain professional relationships with patients and team members.
  • Competence in patient education, research principles, and quality improvement processes.
  • Experience with geriatrics and managing chronic conditions preferred.
  • Participation in or facilitation of quality assurance projects and committee activities.
  • Supervision will be provided through on-site physician oversight, consultations, and chart reviews.
  • Must meet CMS competencies for independent practice and be medically cleared for communicable diseases with up-to-date immunizations.
  • Ability to perform duties in variable conditions, including driving within the catchment area and working in diverse environments.
  • Must be able to perform physical activities such as walking, bending, and lifting up to 40 pounds.
