The Nurse Practitioner will provide primary care services to senior patients within the Senior TLC program across multiple counties. The role involves performing health assessments, managing chronic conditions, coordinating care, and collaborating with a multidisciplinary team under supervision of the Medical Director. The position includes community-based and facility-based care for geriatric populations, with responsibilities aligned with the PACE model of care.

Responsibilities

• Perform health histories, physical exams, and patient education for senior participants.
• Order, interpret, and follow up on lab tests and diagnostic results.
• Initiate, evaluate, and modify medication and treatment plans within approved protocols.
• Assess, coordinate, and deliver direct participant care in homes, hospitals, and SNFs.
• Request consultations from other healthcare services as needed.
• Collaborate with physicians, nurses, and other team members to support participant care and education.
• Participate in care planning, including setting SMART goals and documenting interventions.
• Attend interdisciplinary team meetings, supervisory sessions, and on-call rotations.

Compensation

• Salary-based compensation with a W-2 tax status.
• Sign-on bonus of $3,000.
• Additional income through a sign-on bonus.

Shift & Schedule

• Variable schedule days and hours.
• Primary care on-call rotation scheduled as 1 week on / 7 weeks off.

Requirements

• Master's Degree or higher as a Nurse Practitioner (NP) or Physician Assistant (PA) from an accredited program.
• NC licensure as an NP and Board Certification (ANCC).
• 3-5 years of medical experience, including 1 year as an Advanced Practice Provider.
• At least one year of experience working with frail or elderly populations.
• Ability to learn, teach, and research; problem-solving and analytical skills.
• Knowledge and skills necessary to treat geriatric patients.
• Must be medically cleared for all communicable diseases and have up-to-date immunizations.
• Ability to work within the scope of practice and adhere to organizational competencies.
• Strong teamwork and communication skills.
• Ability to perform procedures and manage emergency situations.
