Azura Palliative Care and Hospice of WI is seeking a part-time, non-exempt Hospice Nurse Practitioner to provide in-home and facility-based hospice care. The role involves assessment, diagnosis, treatment, and care planning for patients with life-limiting illnesses, supporting families, and collaborating with an interdisciplinary team to deliver compassionate end-of-life care.

Responsibilities

• Provide in-home and facility-based hospice care through assessment, diagnosis, and treatment.
• Conduct physical exams and face-to-face visits to determine hospice eligibility.
• Develop and manage individualized care plans in collaboration with the interdisciplinary team.
• Prescribe medications and treatments as appropriate.
• Educate and support patients and families on symptom management and disease progression.
• Coordinate care with physicians, nurses, social workers, and spiritual care providers.
• Maintain accurate clinical documentation in compliance with regulatory standards.
