Azura Palliative Care and Hospice of WI is seeking a part-time, non-exempt Hospice Nurse Practitioner to provide in-home and facility-based hospice care. The role involves assessment, diagnosis, treatment, and care planning for patients with life-limiting illnesses, supporting families, and collaborating with an interdisciplinary team to deliver compassionate end-of-life care.
