The Nurse Practitioner will conduct comprehensive home risk assessments and adult/geriatric evaluations across Ohio and neighboring states. This full-time, traveling role involves collaborating with healthcare teams to review patient health, medical history, and social factors, with the goal of improving care quality and closing care gaps. The position offers a competitive salary, sign-on bonus, and flexible schedule, primarily working weekdays in community and home settings.

Responsibilities

• Conduct adult and geriatric assessments, including medical history, diagnosis, treatment, and health education, following established protocols.
• Perform home risk assessments and observe patient health in various community settings.
• Collaborate with physicians, case managers, social workers, family members, and other healthcare providers.
• Provide follow-up care and patient education in coordination with primary care physicians.
• Refer patients to specialists or additional services as needed.
• Document clinical findings and care activities accurately and timely.
• Work in diverse environments such as homes and skilled nursing facilities.
• Travel extensively within Ohio and bordering states, including overnight travel and weekends as required.

Compensation

• Salary range: USD 113,700.00 to USD 133,500.00 annually, commensurate with experience.
• Compensation model includes salary plus incentives.

Shift & Schedule

• Monday through Friday, dayshift hours (8:00 AM to 5:30 PM).
• No on-call, evening, or weekend hours unless desired.
• Full-time schedule with flexible hours within the standard workweek.

Requirements

• Master's Degree in Nursing or equivalent experience with NP licensure.
• Current RN and Nurse Practitioner licensure in the practicing state, including prescriptive authority or ability to obtain it.
• Board certification by AANP, ANCC, or AACCN in an approved specialty.
• Current BLS, ACLS, or CPR certification.
• At least 1 year of active Nurse Practitioner experience.
• Proficiency with electronic health records and medical software.
• Excellent verbal and written communication skills.
• Willingness to travel up to 80-90% of the time, including overnight and weekend travel.
• Valid driver’s license and automobile insurance coverage.
• Ability to adapt to frequent changes and work independently in community settings.
