Neighborhood Health is seeking a Certified Nurse Practitioner - Family Practice to serve as a primary care provider in an outpatient clinic setting. The role involves delivering comprehensive care to patients of all ages, including conducting physicals, ordering tests, prescribing medications, and patient education. The position is part of a Patient Centered Medical Home (PCMH) team focused on improving access, quality, and patient self-management.

Responsibilities

• Act as the patient's primary healthcare provider in an outpatient clinic setting.
• Conduct physical examinations and order diagnostic tests.
• Educate patients on preventive care and prescribed treatments.
• Prescribe medications within prescriptive authority.
• Address barriers to patient care and coordinate with the PCMH Care Team.
• Collaborate with team members to achieve patient care goals.
• Work full-time, Monday through Friday, during daytime hours.
• Maintain compliance with all organizational policies and standards.

Shift & Schedule

Requirements

• Current certification as a Nurse Practitioner in Family Practice.
• Ability to prescribe medications within scope of practice.
• Experience providing primary care in an outpatient setting preferred.
• Strong communication and patient education skills.
• Commitment to serving underserved populations.
• Ability to work collaboratively within a team environment.
• Must disclose any background issues, including arrests or related matters.
• Willingness to adhere to organizational policies regarding background disclosure.
• Valid state licensure and prescriptive authority.
• Experience working in a Federally Qualified Health Center (FQHC) setting is a plus.
