The Nurse Practitioner at Heart to Heart Hospice provides comprehensive palliative and end-of-life care to patients with life-limiting illnesses. The role involves patient and family education, care planning, collaboration with physicians, and ensuring compliance with legal and regulatory standards. This is a full-time, non-management position requiring clinical experience in hospice or home health settings.

Responsibilities

• Consult with and educate patients and families regarding disease processes, self-care, and end-of-life care.
• Assist in developing patient and family care plans in collaboration with the interdisciplinary team to ensure quality care.
• Interact with attending and consulting physicians regarding palliative care in collaboration with hospice physicians.
• Inform physicians and staff of changes in patient needs and outcomes, and evaluate patient and family responses to care.
• Ensure timely and accurate documentation of all patient interactions and care activities.
• Perform other duties as required to support patient care and team functions.
• Ensure compliance with local, state, and federal laws, Medicare regulations, and organizational policies.
• Provide ongoing patient and family support and education throughout the care process.

Compensation

Requirements

• Graduate of a Baccalaureate School of Nursing.
• Current State license as a Registered Nurse and as an Advanced Practice Nurse (APN).
• Current Driver's License, reliable transportation, and valid auto liability insurance.
• At least two years of experience as an APN in a clinical care setting, hospice, or home health preferred.
• Comprehensive knowledge of pharmacology, palliative care, and end-of-life issues.
• Experience in patient education and care planning in hospice or home health settings preferred.
• Ability to collaborate effectively with physicians and interdisciplinary teams.
• Strong communication skills and compassionate approach to patient care.
• Knowledge of compliance standards related to healthcare regulations and Medicare.
• Ability to perform documentation accurately and timely.
