Continuum Hospice is seeking a Per Diem Hospice Nurse Practitioner to provide comprehensive end-of-life care in Marin and Sonoma Counties. The role involves evaluating patient health, developing care plans, and collaborating with interdisciplinary teams to ensure holistic patient support under physician supervision.

Responsibilities

• Provide comprehensive medical care, manage terminal illnesses, and ensure compliance with the state Nurse Practice Act under physician supervision.
• Consult with attending physicians, hospice medical director, and healthcare practitioners to assess eligibility and coordinate ongoing care.
• Review clinical information and oversee patient care in collaboration with the hospice interdisciplinary team.
• Educate and counsel patients and families on medical, psychological, and spiritual concerns, and work with the team to meet care goals.
• Contribute recommendations regarding patient eligibility for initial certification and recertification for hospice services.
• Maintain current knowledge of Medicare/Medicaid, state, and federal hospice regulations to ensure compliance.
• Collaborate with healthcare team members to deliver patient-centered hospice care.
• Perform all duties in accordance with applicable laws and regulations.

Compensation

• Paid $150 per patient visit.
• Fee-for-Service compensation model.
• W-2 tax status.

Requirements

• Graduation from an accredited School of Nursing.
• Completion of an accredited Nurse Practitioner Program or certification by a recognized national organization meeting California standards.
• Current nursing license in California and CPR certification.
• Master's degree with at least one year of Nurse Practitioner experience and one year of hospice or palliative care experience.
• Certification from an approved certifying body for advanced practice nursing.
• Optional: Certification in a specialty area such as Hospice and Palliative Nursing (CHPN), Pediatrics, or Geriatrics.
• Excellent verbal and written communication skills.
• Proficiency in computer use, including electronic medical records.
• Must be fully vaccinated against COVID-19 or have a documented exemption, per CMS requirements.
