St. Luke's Health System in Meridian, Idaho, is seeking an Advanced Practice Provider to join their Heart Failure Clinic. The role involves managing complex heart failure patients through a collaborative, team-based approach, with a focus on chronic disease management and advanced treatment options. Responsibilities

  • Manage day-to-day care of patients with heart failure, including initial workup and chronic disease management.

  • Participate in discussions regarding advanced heart failure treatments and end-of-life care planning.
  • Coordinate care with the Advanced Heart Failure Team from Intermountain Hospital.
  • Work within a team-based care model alongside four APPs and one supervising physician.
  • Perform patient assessments and develop treatment plans in accordance with clinical guidelines.
  • Contribute to patient education and support for complex cardiac conditions.
  • Maintain documentation and ensure compliance with clinical protocols.

Compensation

  • Salary commensurate with experience.
  • CME allowance included as part of compensation.

  • Knowledge of heart failure management and treatment options.
  • Ability to work Monday through Friday, 10-hour days.
  • Commitment to professional development and ongoing education.
  • Experience working with a team in a hospital or outpatient setting preferred.
  • Proficiency in documentation and electronic health records.
