The Nurse Practitioner or Physician Assistant will provide advanced clinical care to chronically ill geriatric participants within the LIFE program. Under supervision of the Primary Care Physician and Medical Director, the provider will perform assessments, manage care, and collaborate with an interdisciplinary team to improve health outcomes for seniors in the community.

Responsibilities

• Practice within the scope defined by state laws, including physician oversight, practice authority, and prescriptive authority.
• Collaborate with the Primary Care Physician, Medical Director, and interdisciplinary team to deliver participant care and ensure clinical outcomes.
• Coordinate with the Clinic Nurse Manager on operational and documentation issues such as scheduling and charting.
• Perform health histories, physical exams, and patient education.
• Order, interpret, and follow up on lab tests and results.
• Initiate emergency measures when necessary and recognize abnormalities requiring immediate referral.
• Assess, initiate, and coordinate patient care daily under physician supervision.
• Participate in staff meetings, care planning, and quality improvement initiatives.

Shift & Schedule

• Standard hours are 8:00 am to 5:00 pm, Monday through Friday.
• Specific work hours will be established through discussions with the supervisor.

Requirements

• Licensed as a Certified Nurse Practitioner or Physician Assistant in Pennsylvania.
• Minimum of one year experience working with the frail and elderly population or willingness to undergo extensive training.
• Valid State driver's license with a clean driving record.
• Successful completion of background check, physical, drug screening, and TB screening.
• CPR and First Aid certification.
• Ability to make independent decisions and process information for patient safety.
• Excellent written and verbal communication skills.
• Ability to learn, teach, and conduct research; problem-solving and analytical skills.
• Willingness to participate in overnight training or travel as needed.
• Physical ability to move intermittently, lift, carry, and support participants, and operate a motor vehicle.
