We are seeking an experienced Nurse Practitioner to join a hospice and palliative care team in Dayton, Ohio. The role involves autonomous patient care within homes, skilled nursing facilities, long-term care settings, and hospice facilities, with travel within a 50-55 mile radius of Dayton. The position is full-time, contract-based, with potential for extension or permanent placement.

Responsibilities

• Provide hospice and palliative care to patients in their homes, SNFs, LTCs, and hospice facilities.
• Work independently to assess, diagnose, and develop care plans for patients.
• Manage patient care in accordance with hospice and palliative care standards.
• Travel within a 50-55 mile radius of Dayton to patient locations.
• Maintain accurate documentation of patient encounters and treatments.
• Collaborate with interdisciplinary team members to ensure comprehensive patient care.
• Adhere to all licensing, certification, and safety requirements.
• Participate in scheduled shifts, primarily Monday through Friday, 10-hour days.

Requirements

• Active Ohio Nurse Practitioner license.
• Board certification in hospice or palliative care (AANP or ANCC).
• DEA registration and BLS certification (AHA hands-on).
• Valid driver's license and auto insurance.
• Minimum of 2 years of recent hospice or palliative care experience.
• Comfortable working autonomously in patient homes and healthcare facilities.
• Excellent communication and clinical assessment skills.
• Ability to travel within specified radius and manage a flexible schedule.
• Availability to start within 2-3 weeks of offer.
• Experience working in long-term care, home care, or hospice settings preferred.
