The Skin & Wound Care Provider, typically a Nurse Practitioner or Physician Assistant, will lead multidisciplinary wound care teams, perform bedside wound assessments and procedures, and collaborate with medical leadership to deliver high-quality skin and wound care services in the Greater Salt Lake City area. The role involves travel to scheduled locations for wound rounds and house calls, with a focus on improving patient outcomes and wound healing progress.

Responsibilities

  • Lead and develop collaborative multidisciplinary wound care teams
  • Travel weekly to assigned locations for wound rounds and house calls within the region
  • Provide high-quality skin and wound care services to patients
  • Order, interpret, and evaluate laboratory and diagnostic studies to assess patient needs
  • Perform bedside wound care including assessment, dressing application, pressure relief, pain and odor management, and minor surgical procedures
  • Track wound healing progress and document in the EMR promptly
  • Develop individualized treatment plans based on patient needs and clinical guidelines
  • Collaborate virtually with medical leadership including physicians and specialists in wound care, dermatology, podiatry, vascular surgery, and infectious disease

Shift & Schedule

  • Full-time, 0.8 to 1.0 FTE
  • Monday through Friday, 6:00 am to 3:00 pm
  • Dayshift hours with no nights, weekends, or on-call responsibilities

Requirements

  • Licensed Nurse Practitioner (NP, FNP, ARNP, APRN) or Physician Assistant (PA-C) in the working state
  • Active DEA license
  • Reliable transportation, valid driver’s license, and current auto insurance
  • Board certification in nursing or physician assistant scope of practice
  • 1-2 years of wound care and/or surgical experience (preferred but not required)
  • Ability to work independently, be punctual, dependable, and professional
  • Strong interpersonal and communication skills, both verbal and written
  • Ability to stand for extended periods
  • Willingness to travel within the assigned region
