The Hospice Nurse Practitioner at Choice provides clinical assessment and palliative care to terminally ill patients in their residences. The role involves collaborating with the interdisciplinary team, educating patients and families, and ensuring appropriate end-of-life care in accordance with hospice criteria. The practitioner must be adaptable to an unstructured environment and communicate effectively with team members and patients.

Responsibilities

• Collaborate with the interdisciplinary team to evaluate and manage palliative treatment for terminal patients.
• Perform face-to-face visits at patients' residences to assess and address care needs.
• Educate patients and families about disease progression, self-care, and end-of-life options.
• Consult with and inform attending and consulting physicians regarding patient care and assessment results.
• Provide clinical assessment findings to hospice physicians after patient encounters.
• Maintain current knowledge in pharmacology and palliative medicine.
• Ensure compliance with hospice criteria and standards.
• Support and train staff as needed in hospice and palliative care practices.

Requirements

• Current state RN license
• Advanced Practice Nurse license
• Valid driver's license
• Valid auto insurance
• AHA CPR certification
• Experience with HomeCare HomeBase preferred
