The Hospice Nurse Practitioner at Choice provides clinical assessment and palliative care to terminally ill patients in their residences. Responsibilities

  • Consult with and inform attending and consulting physicians regarding patient care and assessment results.

  • Maintain current knowledge in pharmacology and palliative medicine.
  • Ensure compliance with hospice criteria and standards.
  • Support and train staff as needed in hospice and palliative care practices.
