Horizon House is seeking a full-time Advanced Practice Nurse Specialist to provide psychiatric and behavioral health services within the Developmental Services Division. The role involves clinical assessments, medication management, crisis intervention, and promoting integrated behavioral and physical health care for adults with psychiatric or developmental disabilities, substance use issues, or experiencing homelessness.

Responsibilities

• Provide clinical assessments, evaluations, medication management, psychiatric treatment, and crisis assessment/intervention.
• Facilitate integration of behavioral and physical health services to promote person-centered health and wellbeing.
• Develop and interpret nursing philosophies and standards of care to ensure quality healthcare delivery.
• Enforce adherence to nursing best practices and measure health outcomes against established standards.
• Develop and maintain training programs for nursing staff.

Requirements

• Current Psychiatric/Mental Health Nurse Practitioner certification or Family Nurse Practitioner certification.
• Unencumbered nursing license in Pennsylvania.
• Minimum of five (5) years of clinical psychiatric and community nursing experience.
• Two (2) years of experience as a Nurse Practitioner preferred.
• Proficiency with Microsoft Word, Excel, PowerPoint, and electronic health records systems.
• Familiarity with telepsychiatry and telehealth platforms.
• Valid driver's license with an acceptable driving record.
• Ability to speak clearly and hear easily.
• Flexibility in scheduling and ability to provide 24-hour on-call coverage.
• Physical ability to lift, carry, kneel, reach, and perform other physical tasks as required.
