The Physician Assistant will provide comprehensive medical care to chronically ill geriatric members within a senior healthcare program. The role involves conducting medical assessments, managing treatment plans, coordinating with interdisciplinary teams, and supporting members' independence in their homes and communities. The position offers a full-time, daytime schedule with no evenings, weekends, or holidays.

Responsibilities

• Perform health histories and detailed medical examinations for new members to establish initial care plans.
• Initiate, evaluate, and review medical orders, identifying opportunities for care improvement.
• Perform procedures and document all care activities and recommendations for team members.
• Order, interpret, and follow up on laboratory tests and results.
• Coordinate with internal and external providers, including physicians, therapists, dietitians, and social workers.
• Educate staff, members, caregivers, and families on essential care functions to promote independence.
• Conduct ongoing assessments to update and improve care plans based on evaluations and member feedback.
