Mobile Physician Services is seeking a part-time licensed Primary Care Nurse Practitioner or Physician Assistant to provide home-based primary care in the Pinellas/Pasco County, FL area. The role involves performing age-appropriate assessments, managing acute and chronic conditions, and collaborating with specialists and local providers to deliver comprehensive care to homebound patients.

Responsibilities

  • Perform age-appropriate history and physical examinations for complex, acute, critical, and chronically ill patients independently.
  • Conduct annual wellness exams.
  • Diagnose and develop treatment plans for homebound patients with acute, episodic, or chronic illnesses based on medical histories, symptoms, physical findings, or diagnostic imaging.
  • Maintain standards of care and adhere to professional practice guidelines.
  • Collaborate with specialists to develop treatment plans for high-risk patients.
  • Partner with local home health agencies, labs, imaging providers, and vendors.
  • Communicate with patients and caregivers regarding self-management of diagnoses.
  • Respond promptly and knowledgeably to patient and caregiver questions.

  • Personable, service-oriented, and professional with excellent communication skills.
  • Strong attention to detail, especially in handling electronic medical records.
  • Preferred: One year of home health experience.
  • Ability to work independently and manage a variable schedule.
