The Primary Care Nurse Practitioner role in Columbus, OH, involves outpatient care for seniors with multiple chronic conditions within a physician-led, mission-driven organization. The position offers a balanced schedule with no nights or weekends, administrative support, and a focus on value-based care for a small patient panel.

Responsibilities

• Manage a patient panel of up to 450 seniors with 3-5 chronic diseases.
• Provide outpatient primary care services in a clinic setting.
• Collaborate with a dedicated care team including medical assistants and schedulers.
• Deliver holistic health services such as acupuncture, Tai Chi, Yoga, and nutrition counseling.
• Participate in value-based care initiatives to improve patient outcomes.
• Coordinate transportation for patients as needed, at no charge.
• Maintain a schedule of Monday through Friday, 7:30 am to 5:00 pm, with the last patient at 4:15 pm.
• Utilize administrative time allocated within the schedule for documentation and care planning.

Compensation

• Salary of $140,000 annually, with potential to increase with partnership.
• Performance bonus of up to $10,000 in the first year, increasing as a partner.
