Community Health Service, Inc. is seeking a part-time Nurse Practitioner or Physician Assistant to provide primary care services in Rochester, MN. The provider will deliver outpatient healthcare, perform procedures within primary care scope, and collaborate with a multidisciplinary team to serve underserved populations. The role emphasizes health equity, community engagement, and team-based care within a nonprofit setting.

Responsibilities

• Provide primary health care to patients at CHSI practice sites.
• Perform outpatient procedures within the scope of primary care.
• Collaborate with physicians, nurse practitioners, behavioral health providers, nurses, and medical assistants.
• Consult with staff and serve as a resource for clinical issues.
• Follow national patient safety goals in patient care.
• Assist in developing and implementing clinical protocols and care plans as needed.
• Establish rapport with community agencies and serve the needs of underserved populations.
• Model effective teamwork and promote positive morale among staff.

Shift & Schedule

Requirements

• NP or PA with 2-3 years of outpatient clinical experience.
• Valid state medical license and DEA registration in Minnesota.
• Basic Life Support (BLS) certification.
• Ability to diagnose and treat conditions within full-spectrum primary care.
• Knowledge of outpatient clinical practice standards and health promotion strategies.
• Strong teamwork skills and ability to establish effective patient and staff relationships.
• Commitment to health equity and cultural humility.
• Physical ability to perform duties, including lifting up to 50 pounds and repetitive motor movements.
• Ability to sit continuously, and stand or walk frequently during an eight-hour shift.
• Desirable: experience in community health, bilingual in Spanish, Somali, Arabic, or Amharic, and understanding of underserved populations' socio-economic and health issues.
