We are seeking a Certified Registered Nurse Practitioner (CRNP), Doctor of Nursing Practice (DNP), or Physician Assistant (PA) to provide primary care services through a hybrid model of telehealth and in-home visits across York, Lancaster, and Harrisburg, PA. The role offers autonomy, flexible scheduling, and the opportunity to serve diverse patient populations in community-based settings.

Responsibilities

• Deliver high-quality primary care via telehealth and in-person visits, including virtual triage and management of acute and chronic conditions.
• Perform in-home house calls and virtual consultations within a 30 to 60-minute travel radius.
• Build and maintain strong patient relationships through effective communication and personalized care.
• Manage a diverse caseload with a focus on patient-centered outcomes.
• Coordinate care and ensure timely interventions for urgent and non-urgent health issues.
• Document patient encounters accurately in electronic health records.
• Participate in a collaborative team environment focused on quality improvement.
• Maintain licensure, certifications, and compliance with healthcare regulations.

Compensation

• $120,000 – $150,000 yearly salary (W-2)
• Compensation includes mileage reimbursement at the current IRS rate.

Requirements

• Current CRNP, DNP, or PA licensure with experience in primary care or family medicine.
• Experience in urgent care or emergency medicine is a plus.
• Self-motivated with the ability to work independently in diverse settings.
• Excellent communication and patient engagement skills.
• Comfortable with virtual care platforms and in-home visits.
• Ability to manage both acute and chronic health conditions.
• Reliable transportation for in-home visits within the designated travel radius.
