The Nurse Practitioner will conduct comprehensive health assessments and provide medical care to adult and geriatric patients in home, skilled nursing facility, and community settings in Mansfield, Ohio. The role involves collaborating with healthcare team members, reviewing patient health, and documenting findings to improve care quality and address care gaps. This is a full-time position with flexible hours, travel required, and opportunities for incentive compensation.

Responsibilities

  • Conduct adult and geriatric health assessments, including medical history, diagnosis, treatment, and health education.
  • Review and observe patients' current health status, medication adherence, social environment, and risks during visits.
  • Collaborate with physicians, case managers, social workers, family members, and other healthcare providers.
  • Provide follow-up care and coordinate referrals to physicians or case management as needed.
  • Work in various settings including home visits and skilled nursing facilities.
  • Document clinical findings and care plans according to organizational protocols.
  • Participate in patient education and support care management efforts.
  • Travel to meet patients in their homes or community environments, including overnight travel as required.

Compensation

  • Salary range: USD 103,700.00 to USD 123,500.00 annually.
  • Compensation model includes salary plus incentives.
  • Additional earning opportunities through monthly and annual incentives.
  • Sign-on bonus of $7,500.

Shift & Schedule

Requirements

  • Master's Degree in Nursing or equivalent experience with NP licensure.
  • Current RN and Nurse Practitioner licensure in Ohio, including prescriptive authority or ability to obtain it.
  • Board certification by AANP, ANCC, or AACCN in an approved specialty.
  • Current BLS, ACLS, or CPR certification.
  • At least 1 year of Nurse Practitioner experience preferred; new graduates encouraged to apply.
  • Strong computer skills and familiarity with electronic medical records.
  • Excellent verbal and written communication skills.
  • Willingness to travel up to 15%, including overnight stays, with a valid driver’s license and adequate auto insurance.
  • Ability to adapt to frequent changes in work environment.
