The Greater Lawrence Family Health Center is seeking a bi-lingual (Spanish/English) Nurse Practitioner to provide primary care services within a community health setting. Responsibilities
- Assess patients by reviewing medical history, presenting problems, lab results, allergies, psychosocial factors, medications, and living arrangements.
- Review abnormal lab values and plan appropriate treatment or disposition in collaboration with the primary care team.
- Identify patient and family education needs, develop teaching plans, and provide information on wellness, illness, and rehabilitation.
- Participate in a multidisciplinary team approach, consulting with other healthcare providers and involving family members as appropriate.
- Provide care following approved protocols, evaluate patient responses, and adjust treatment plans accordingly.
