Gather Health is seeking a Board-Certified or Board-Eligible Primary Care Physician in Internal Medicine or Family Medicine to provide comprehensive, person-centered care for older adults. The role involves in-clinic and telehealth services supported by a dedicated care team, with a focus on addressing social determinants of health and promoting community wellness. The physician will collaborate with multidisciplinary team members to deliver high-quality, longitudinal care tailored to patient goals in a mission-driven organization.
Responsibilities
- Participate in daily team huddles to discuss patient concerns and care plans
- Perform in-person exams and telehealth visits supported by EMTs in patients' homes
- Collaborate with 'Gather Guide Partners' to address patients' social needs
- Communicate and coordinate with emergency room providers regarding patient history and care
- Engage with patients to support their health journey and provide ongoing communication
- Document clinical visits and notes in the Athena EHR system
- Provide on-call support for escalated cases

Requirements
