Prosano Health Solutions is seeking a Nurse Practitioner or Physician Assistant to join our primary care team. The provider will evaluate and treat patients, coordinate care services, participate in team activities, and contribute to continuous improvement efforts within a patient-centered, value-based care environment. This role requires on-site work at our care center in Arizona and involves collaboration with a multidisciplinary team to deliver high-quality, personalized healthcare.

Responsibilities

• Evaluate and treat patients in accordance with standards of care.
• Coordinate patient services, including referrals, hospital and SNF coordination, durable medical equipment, and home health care.
• Participate in daily team huddles and lead care team activities as needed.
• Partner with the care team to promote a healthy care culture.
• Complete all medical record documentation in a timely manner.
• Follow policies and protocols established by Clinical Leadership.
• Meet regularly with physicians and care team leaders to review quality metrics and update policies.
• Engage in community relationship building with healthcare providers and organizations.

Requirements

• Successful completion of a Nurse Practitioner or Physician Assistant program with Board Certification.
• Active, unrestricted Nurse Practitioner or Physician Assistant license in Arizona.
• Minimum of two to five years of relevant clinical experience preferred.
• Excellent written and verbal communication skills.
• Willingness to adapt to a value-based care setting.
• Basic computer skills, including email and EMR systems.
• Certification to prescribe medications under a collaborating physician or medical director.
• BLS certification.
• Active AZ fingerprint clearance card.
• Active DEA license.
