GuideStar Eldercare is seeking a Nurse Practitioner to provide behavioral health services to patients with neurocognitive disorders in nursing homes and assisted living facilities. The role involves autonomous clinical practice with support from corporate leadership and administrative staff, focusing on patient safety, functionality, and dignity within a multidisciplinary team.

Responsibilities

• Travel daily to assigned facilities from home to provide care.
• Conduct initial evaluations, ongoing therapy, and cognitive testing for patients.
• Document clinical needs and service details for each patient.
• Develop and maintain professional relationships with facility staff, patients, and families.
• Collaborate with a multidisciplinary team to promote patient safety and well-being.
• Ensure compliance with clinical documentation and regulatory requirements.
• Participate in ongoing training and professional development activities.
• Adhere to all facility and organizational policies and procedures.

Compensation
