Gather Health is seeking board-eligible or board-certified Primary Care Physicians (Internal Medicine or Family Medicine) to provide comprehensive, person-centered care to older adults in a primary care setting. The role involves in-person and telehealth visits supported by a multidisciplinary team, with a focus on addressing social determinants of health and promoting community wellness. Responsibilities

  • Participate in daily team huddles to discuss patient concerns and care plans
  • Perform in-person exams and telehealth visits supported by EMTs in patients' homes
  • Collaborate with care team members, including 'Gather Guide Partners,' to address social needs
  • Communicate and coordinate with emergency room providers regarding patient history and care
  • Engage with patients to support their health journey and provide ongoing communication
  • Document clinical visits and notes in the Athena EHR system
  • Provide on-call support for escalated cases

  • Salary plus performance incentives, including performance bonuses

Requirements

  • Board certification or eligibility in Internal Medicine or Family Medicine (must obtain certification within one year of start date)

  • Ability to effectively communicate with patients from diverse backgrounds
  • Experience working with EHR systems and relevant healthcare technologies
  • Strong ethical standards and ability to handle sensitive information confidentially
  • Commitment to a mission-driven organization focused on community health and equity
  • Ability to work collaboratively in a non-hierarchical team environment
  • Ability to remain calm and prioritize effectively in a dynamic setting
