The Physician Assistant (PA) will provide primary care services within the VA Southern Oregon Rehabilitation Center & Clinics, functioning as part of a Patient Aligned Care Team or as a Float Practitioner. The role involves managing a panel of patients with complex and chronic health issues, delivering evidence-based care, and collaborating with interdisciplinary teams to meet medical and psychosocial needs. The position emphasizes community-based outpatient care and patient education, with a focus on health maintenance, prevention, and wellness.

Responsibilities

  • Manage a panel of patients with complex and chronic health conditions, following evidence-based guidelines.
  • Develop and maintain an interdisciplinary approach to patient care within the primary care team.
  • Order diagnostic and therapeutic tests appropriate to patient age and health status.
  • Prescribe pharmacologic and nonpharmacologic interventions to support patient health.
  • Monitor and document treatment effectiveness and progress toward clinical goals.
  • Initiate referrals and consults to appropriate services as needed.
  • Provide patient and family education on medical, social, and behavioral health issues, emphasizing self-management and prevention.
  • Collaborate with team members to identify and coordinate resources to address patient needs.

Compensation

  • Salary commensurate with experience.
  • Additional income opportunities include Loan Repayment, Relocation Support, and Sign-on Bonus.
  • Relocation assistance and loan repayment options are available.
  • Compensation is provided via W-2 tax status.

  • Monday through Friday, 8:00 AM to 4:30 PM.

Requirements

  • Certification as a Physician Assistant (PA) eligible for practice in Oregon.

  • Ability to independently manage a panel of patients with complex health issues.
  • Knowledge of evidence-based guidelines and population health management.
  • Strong collaboration and communication skills within interdisciplinary teams.
  • Ability to order and interpret diagnostic tests and prescribe appropriate treatments.
  • Commitment to patient education and health promotion.
  • Understanding of VA healthcare standards and policies.
  • Ability to work Monday through Friday, 8:00 AM to 4:30 PM.
